| Very possible, the first
impression any trucking company considering your
application will get of |
| you, will be your DAC
services report. DAC's simply a consumer reporting
agency. DAC does |
| not create
information about you. DAC only collects
information
former employers offer and |
| make the information
available to USIS member companies. Inaccurate/
harmful information |
| about you, is responsibility of
trucking company who created / submitted it, NOT DAC
services. |
| |
| DAC, is not required
to verify, or held responsible for accuracy of
report information supplied |
| by former employer
until a truck driver disputes the information DAC
reports on the driver. |
| |
| Simple unintentional
mistakes, human errors are most common.
Typically starting or ending |
| employment dates, addresses,
license number, etc, are not consistent with current
application. |

WHEN should you
check your dac report
|
| It's crucial to every
truck driver to know what is contained on their dac
report. Your information is |
| entered by hand by your
former employers and incorrect information (although
considered human |
| error) can hurt (even
eliminate) your new truck driver job opportunity. |
|
| It's your
responsibility truck driver. You must take the time
to obtain and review your dac |
| report to make sure
all information's
correct and have necessary
corrections made if it is not. |
|
|
|
Keep in mind, your dac report's the first thing a
new employer is going to see considering "you" |
|
for that truck driving position you want. Monitoring
your dac report is the critical to your trucking
|
| career management.
Check immediately if you haven't yet and at least once a
year from now on. |
|
| Try and make sure your
dac report presents an accurate, fair expression of all
past employment. |
|

get a copy of your dac report
|

YOU ARE ENTITLED TO
receive a NO-COST copy of
your dac report once every twelve months.
|
|
Additional copies may be provided
within a twelve month period, subject to a $9.50
charge. |
| |
|
There are circumstances under which you
may be entitled to additional copies at no cost.
If |
| for instance, you
were denied employment based on information placed on
your dac report, |
|
you'd be entitled to a current copy at no
charge. Simply request a copy of that dac
report. |
|
|
| Remember
USIS (DAC Services) only has information available on
drivers who have applied for, |
| or
been employment by those trucking companies who are their
customers. You should inquire |
|
(to confirm you have a report on file) about
your dac report information by calling (800) 381-0645. |
|
|
| You must however
request and receive your actual dac report by mail only.
To do that |
| you need to send a
letter requesting your dac report and include
the following information. |

If, you have received
your dac report and found an entry to be incorrect,
contact the employer
|
|
who
provided that inaccurate information. They may be
willing to correct it immediately. |
| |
|
If employer will not correct it
you'll need to contact DAC
Services and formally dispute it. |
| |
|
Write a letter stating your
reasons for disputing information you feel to be incorrect
and request |
| the information be either
corrected or removed. Be the
professional that your are. Keep it simple,
|
| courteous
and to the point. Send copies of any
documentation you may have supporting
your |
| request. Copies of
ticket, log book, pay stubs, receipts, anything that may
help prove your point. |
| |
| Send the
request by Certified Mail
with Return Receipt. |
| |
| DAC
then has thirty days to verify the
information is correct.
A copy will be forwarded to the |
|
company along with all other
supporting documents you may have included. If
company can not |
| verify
the item in question as correct DAC is required, by the
FCRA, to correct
or remove it. |
|
|
If the company does verify
information's correct, it will not be removed from the
report. |
|
If you do not
agree, still feel information is false or inaccurate,
you have two options. |
| |
| Get an attorney, or,
you have the right to submit a rebuttal statement. |
| |
| In
fact, you have a right to submit a rebuttal statement
for each incorrect entry on your dac |
| report.
If information's partially correct, but you
feel it's unfairly negative for blaming you, submit |
| a rebuttal. Your
"rebuttal" is simply your statement of the
actual facts, as you see them. |
| Your
rebuttal will then be placed on your dac report, along
with that employers information. |
|
| However,
too
many rebuttals on a report may appear worse than a few small negatives. |
